Executive Summary

The Medicaid Early and Periodic Screening, Diagnostic, and Treatment program requires each state to complete a reporting form provided by the U.S. Department of Health and Human Services Centers for Medicare & Medicaid Services (CMS). This form, known as the Form CMS-416, measures the states’ efforts to ensure that eligible children receive the services provided through EPSDT. The forms are collected each year by the federal government.

This chart book presents Form CMS-416 data from the states for Federal fiscal years 2015-2019 as reported by CMS.
