Executive Summary

The Medicaid Early and Periodic Screening, Diagnostic, and Treatment program requires each state to complete a reporting form provided by the U.S. Department of Health and Human Services Centers for Medicare & Medicaid Services (CMS). This form, known as the Form CMS-416, measures the states’ efforts to ensure that eligible children receive the services provided through EPSDT. The forms are collected each year by the federal government.
